The context error with cancel code is typically for user cancellation which
should be at debug level. For other error codes we should display a warning.

v3rpc: return leader loss error if lease stream is canceled

Canceling the stream won't cancel the receive since it's using the internal
grpc context, not the one assigned by etcd.

Would retry a few times before returning a not primary error that
the client should never see. Instead, use proper timeouts and
then return a request timeout error on failure.

Basic support for lease operations like create and revoke.
We still need to:
1. attach keys to leases in KV implmentation if lease field is set
2. leader periodically removes expired leases
3. leader serves keepAlive requests and follower forwards keepAlive
requests to leader.
